What the service may involve
We clarify the concern, urgency, location, funding, consent and which discipline is likely to be appropriate.
The clinician gathers relevant history, observes function and completes suitable clinical measures.
Goals, treatment, education, equipment or environmental recommendations are agreed as appropriate.
Progress, risks and recommendations are reviewed and communicated according to consent and the service agreement.
Working with the broader team
With consent, clinicians may educate family members or support workers, clarify safe implementation and provide written recommendations. This helps maintain consistency without shifting clinical responsibility to people outside their role.
Reports and documentation
If a report, support letter, funding document or detailed care-team communication is required, we will clarify the intended audience, scope, fee and timeframe before commencing where possible.
