Feedback and complaints
Help us understand what went well and what needs attention.
We welcome respectful feedback and aim to respond fairly, protect privacy and use concerns to improve our service systems.
Your experience matters
Compliments, suggestions, concerns and complaints are all welcome.
Feedback helps us understand what is working, what could be improved and whether a matter requires timely action. A person should be able to raise a concern without fear that it will negatively affect access to appropriate service.
You may provide feedback yourself or through an authorised representative. We may need to confirm identity, authority and consent before discussing personal details.
What we aim to do
- Acknowledge the issue and clarify what happened
- Consider immediate safety or service risks
- Keep the person informed about next steps
- Document outcomes and improvement actions
- Explain relevant external review options where appropriate
How to raise feedback
Use the method that feels most practical.
You can use the form below, contact the approved public phone or email once displayed, or speak with a team member. Tell us whether you are seeking a response and how you prefer to be contacted.
Please provide only enough information for us to understand and follow up. Do not include detailed health records in the web form.
Urgent safety
Use emergency or specialist pathways when needed.
For immediate danger or a medical emergency, call 000. The website form is not continuously monitored and is not an emergency reporting channel.
Where a concern relates to a particular funding program or regulator, we can provide information about relevant external complaint or review options.
Send feedback
Tell us what happened and what response you are seeking.
Select “Feedback or complaint” in the enquiry type field. Avoid detailed clinical records or identity documents.
